A quick guide on creating surveys for customer feedback

Here’s how to create a survey using the SurveyMonkey tool which is free at www.surveymonkey.com.  Click the CREATE SURVEY button at the top right. There are three options to select:

1. Create new survey
2.  Copy existing survey
3.  Use Survey template

Click ADD QUESTION HERE on the page where you want the question displayed. The QUESTION EDITOR window will open. Select the QUESTION TYPE from a drop-down window. This gives you a choice of text fields so that you can customize text, question choice, and answer choices.

There are also options for how the questions behave for respondents and how they look. These options can be found by scrolling on the left side for the EDIT QUESTION window. Click ADD PAGE BEFORE/AFTER if you want to add another page. When the design process is complete, the survey is ready to send to your audience.

This is an online application so the respondents will have to access it by way of a URL or link. To get a link for this, you must create a COLLECTOR. SurveyMonkey offers quite a few collection systems which are determined by the way you wish to collect the data:

• Using social networks
• Embedded pop-ups
• Track identity
• Anonymously
